Get AnIllinois Medical Cannabis Card

Step 1: ELIGIBILITY

If you live in Illinois and you have a qualifying medical condition, you are eligible to apply for your medical cannabis card. You may click here to see the qualifying medical conditions.

Step 2: PHYSICIAN RECOMMENDATION

The next step is to obtain your doctor recommendation. When discussing medical cannabis with your doctor, it’s important to be honest about your health situation and explain why you believe medical cannabis treatment will be beneficial for you. If your doctor agrees to give you a medical cannabis recommendation, they will need to complete a Physician Certification Form. Once completed, the physician certification will be provided to you. If you are a veteran receiving care at the VA, you will need medical records instead of a doctor recommendation. Instructions for veterans are available here. Patients under the age of 18 will require an additional certification from a second doctor.

Step 3: PATIENT APPLICATION

Once your doctor has completed your patient certification, the next step is submitting your patient application. You can complete the application form by clicking here and creating an account. Be sure to fill out all sections of the form, include the required documents, and make payment of your application fee. Additional instructions on completing the application are available here. Please note that the application process for patients under 18 is different and can be reviewed here.

Important Note: Be sure to select Columbia Care Illinois as your dispensary when filling out your application. Simply enter the following in the “Medical Cannabis Dispensary Selection” section:

Once your application has been submitted, the Illinois Department of Public Health must review it within 30 days. If your application is approved, your Illinois medical cannabis card must be issued to you within 15 days.

Step 4: MAKING PURCHASES

Once you’ve received your medical marijuana card, you’re welcome to visit the dispensary and make purchases. Our staff will be happy to assist you in selecting the best products for your specific condition and symptoms. We look forward to seeing you at the dispensary!
