Step 1: Eligibility
If you live in Arizona and you have a qualifying medical condition, you are eligible to apply for your medical cannabis card. Patients diagnosed with any of the following conditions are eligible to apply for a card:
- Chronic pain
- PTSD
- Crohn’s disease
- Glaucoma
- Hepatitis C
- Severe nausea
- Severe or persistent muscle spasms
- Alzheimer’s disease
- Cachexia or wasting syndrome
- Cancer
- HIV/AIDS
- Seizures
Step 2: PHYSICIAN RECOMMENDATION
In order to receive your medical marijuana card, you’ll need to begin by obtaining a physician recommendation. There are many doctors currently approving patients for medical marijuana in Arizona. When discussing medical cannabis with your doctor, it’s important to be honest about your health situation and explain why you believe medical cannabis treatment will be beneficial for you.
Step 3: PATIENT APPLICATION
Once you’ve received your doctor recommendation, you’ll need to submit a patient application to the AZ Dept. of Health Services. Click here to access the instructions and online application. Medical cards are mailed to the patient within 5 business days of receiving a complete application.
Step 4: MAKING PURCHASES
Once you’ve received your medical marijuana card, you’re welcome to visit the dispensary and make purchases.
